If it were a resume I would have left it out, but since it is on the application you should have included it. I'd be willing to bet they fire you over it if they ever find out. Every company I have worked for has done that regardless of how long someone worked there or I how good of an employee they are. It's basic ethics, you will have proved to be untrustworthy

that's what I worry about the most. I don't like to take any risks when it comes to matters such as this. I'd almost rather not get a job and be completely honest than to lie (or omit information) and get a job on those pretenses.
