Ray Lewis was on the cover of Madden 05. In 2005, Ray Lewis got hurt and only played 6 games

Drew Brees was on the cover of Madden 11. In 2011, the Saints got knocked out of the first round of the 2010 playoffs after being heavily favored.

Forgot about Ray, but individually Brees had a good season (gag) his defense let him down.

Not sure about Ray and Drew, but Larry was saved by Troy Polamalu(sp?). They shared the cover, and IIRC Troy missed around half the season with some injury. Madden can only hit one guy I think.
