Well there are ads shown by Third Party Applications, then there are ads shown by Facebook.

You can choose to restrict showing your actions in Facebook ads, but I dont know if you can tell them not to use your information.

You do know that when you ignore/deny their request....it doesn't tell them, right? It doesn't send them some message like "TheSaint hates you and refuses to be your friend"

So, unless you have people stalking you....they probably won't realize you didn't accept. Especially if it is someone you haven't seen in 10 years.
