Who's to blame for Cam's injury?

Yes he could throw it away. They won't call grounding if you throw it well beyond the line and there is anyone remotely around the spot. Plus he wasn't about to be hit, he was running around and could have easily chucked it. The problem is you should face the blitz and throw into it as the receiver should be wide open just like Allen did on that blitz later. Cam turns his back and starts bailing out which is exactly the opposite of what you should do. He is terrible under pressure and teams are going to continue to blitz until he beats it consistently .

Exactly.

If you want to stop teams from blitzing you have to beat them when they do it.  What Cam needs to realize is that "beating" a blitz doesn't necessarily mean gashing them for 30+ yards.  Sometimes beating the blitz means taking a 3 yard slant and letting the receiver make what he can in the open field.
