Just for arguments sake, a solution to the ref problem. Make all plays and penalties part of the "challenge' protocal.The same number of challenges per team ( or possibly add one), with the same penalty for losing the challenge.

Would not add time to the game (unless you add one challenge, which would be minor)  whIle giving an option to rectify some of these awlfull calls.  What do you think?
